Pre-Visit Check List:
Please help us to help you with your insurance claim by bringing and ID with a photograph such as a drivers license and a valid insurance card with all plan and group numbers and effective dates visible. If your insurance requires a referral from your primary care physician please check with their office to be sure that it has been done.
Please try to obtain copies of doctors notes and x-rays related to your current problem for which you wish to see Dr. Savage. If your x-rays were done at Austin Radiological Association, River Ranch Radiology, St. David’s Hospital network, or Seton Hospital Network, or Austin Regional Clinic the films can be obtained by Dr. Savage’s office via secure internet servers and you do not need to bring copies of films done at these locations.
